Accepted Insurance
Pacific Spine & Rehab participates with auto-related and workers' compensation insurance. We do not participate with most commercial health insurance plans (PPO, HMO, Medicare, Medi-Cal, TRICARE).
This is a deliberate choice. Insurance restrictions on chiropractic care often dictate short visits and limited treatment options, which we have found does not produce the outcomes we want for our patients. Self-pay and lien-based care give us the room to treat properly.
- First-Party Auto Insurance (PIP / MedPay) — accepted
- Third-Party Personal Injury (PI) — accepted on lien
- Workers' Compensation — accepted with authorization
- Commercial health plans (PPO/HMO/Medicare/TRICARE) — not currently in network
- Self-pay with $49 New Patient Special and transparent rates
New Patient Forms
After you book, we email a short intake form with a secure link. Completing it before your visit saves about 10 to 15 minutes in the office.
If you prefer to fill out paperwork in person, plan to arrive 15 minutes before your appointment time. The front desk will provide the same forms on a tablet.

Payment Options
We accept most common payment methods. Self-pay patients can use a card, ACH, HSA/FSA, or CareCredit, and we offer payment plans for longer care arrangements.

Billing Questions
Our front desk handles billing directly — you do not go through a third-party billing service. For PI, MVA, and WC patients, billing goes to the relevant insurance carrier or attorney's office on lien, so you do not see a statement during active care.
If you have a question about a charge, a receipt, or how a payment was applied, call either office and we will resolve it on the spot.
What to Bring to Your First Visit
Bringing a few items helps the visit run faster and gives the doctor the full clinical picture.
- Government-issued ID
- Insurance card if you are using auto, PI, or WC coverage
- Claim number and adjuster contact for active claims
- Attorney contact if you have legal representation
- Any recent imaging (X-ray, MRI, CT) on disc, portal link, or phone
- List of current medications and supplements

If you have a PI, MVA, or WC claim and want to confirm coverage before you book, call either office with your claim number and adjuster contact. We will verify benefits and call you back within one business day.
